State(s):

The targeted data is available for only states within US. Choose any number of states from the list box. At least one state must be chosen. Remember, you can get maximum 100,000 records at one time.

County(s):

Click the link "Populate Counties" to see the list of state wise counties from the selected state(s). You can choose any number of states and their counties from the list. This will populate the zip codes in those counties automatically in Zip Code(s) list box. If you leave the State(s) unselected, the link "Populate Counties" can not be used.

Distance Radius:

After choosing the State, Enter Radius (in miles) and From Zip Code fields. The list of zip codes that are matching the distance criteria will be populated in Zip Code(s). Any old Zip Code(s) contents will be replaced with the new list.

Zip Code(s):

Selected zip codes will be populated here. "Populate Counties" and "Distance Radius" are different tools available to populate the Zip Code(s). Or you can choose to enter the zip codes manually here.

Taxonomy Code(s):

Any number of Taxonomy Codes can be selected. Multiple selections can be done using Ctrl key (OR) Shift key.

Taxonomy Group(s):

Enter the first three characters of a taxonomy code. This will indicate to include all taxonomy codes that start with those characters. You can enter any number of Taxonomy Group(s) separated by commas. Some valid examples are below:

Selection Logics:

     • When you have selections in State(s) and Zip Code(s), only Zip Code(s) list will be used to query the provider records.

          Zip Code(s) AND (Taxonomy Codes(s) OR Taxonomy Group(s))

     • When you have selections only in State(s):

          State(s) AND (Taxonomy Code OR Taxonomy Group)

     • When you have no selections in Taxonomy Code(s) and Taxonomy Group(s),:

          All available Taxonomy Codes will be returned.

Targeted Data Builder will produce the data files in NPIDS normalized data model. It will produce five files (npis.csv, personnames.csv, orgnames.csv, taxonomies,csv and otherids.csv). These are normalized provider data files and are linked together with NPI # as primary key.

Data Structure:

For Detailed Provider Data Structure (Model) of the Targeted Data output, Take your Mouse Over the boxes below and use tab, up and down arrow keys:

     1. Primary Key and Composite Key columns are high lighted in blue color font in diagram/data layout below.
     2. Each record in NPI file (npis.csv) will either have a record in Person Names file (personnames.csv) or in Organization Names
        file (orgnames.csv).
     3. One provider record can have upto 15 records in Taxonomies file (taxonomies.csv). Each record will be identified with sequence
        number from 1 to 15. The combination of NPI number and Sequence number is always unique.
     4. One provider record can have upto 50 records in Other IDs file (otherids.csv). Each record will be identified with sequence
        number from 1 to 50.The combination of NPI number and Sequence number is always unique.

Click "Targeted Data" tab to enter your Targeted Data selection criteria screen.

The provider data in NPIDS data sets are taken directly from monthly updates from the National Plan & Provider Enumeration System (NPPES) from Centers for Medicare and Medicaid Services (CMS), a division of the US Department of Health and Human Services (HHS), an agency of the US federal government. NPIDS is not responsible for the quality and timeliness of NPPES data. NPIDS might add few columns in some of the files to have normalized data and to accommodate derived descriptions of the data, which would help the customers for the practical use of the dataset in their systems and environments.
