  This file contains provider enrollment data for Medicare-enrolled providers. Each record in Entity Type.csv that has the "in_pecos" flag will have corresponding provider enrollment data in this file, allowing you to explore Medicare-specific information.

  To provide addresses, telephone numbers, and fax numbers, each provider's primary practice address, secondary practice address, and business mailing address are included in the Location.csv file.

• Other_Orgname.csv:
